Payment Plan Agreement

This Agreement ("Agreement") is entered into by and between RTD EZ LLC ("C/TPA") and the client ("Client") upon the selection of a payment plan. By checking the box and proceeding with the payment plan, the Client acknowledges they have read and agree to the terms and conditions outlined below:

  1. Payment Plan Options
    The Client agrees to a two-payment option plan, which includes an initial payment and a final payment. The amounts of these payments and the due dates are clearly stated and agreed upon by both parties.

  2. Binding Agreement
    Regardless of the amount of the initial or final payment, the Client agrees to be bound by the terms of this Agreement and is responsible for fulfilling both payments in full as per the agreed schedule.

  3. Non-Completion of Payments
    If the Client fails to make the final payment in full, the SAP is under no obligation to upload any information or update the progress to the Client’s clearinghouse. Services will not be rendered in their entirety until the full payment is made.

  4. Obligation of SAP upon Full Payment
    Once the payment plan is fully paid, the SAP agrees to complete their obligation of updating the SAP program progress to the clearinghouse.

  5. Communication of Payment Issues
    The Client agrees to provide clear and timely communication to SAP if any circumstances arise that may affect the Client’s ability to make payments on time. Failure to communicate such issues promptly may result in delays or discontinuation of services.

  6. Full Payment Required for Services
    Services will not be rendered in their entirety unless the Client has paid the full balance owed under this Agreement. Any partial payments or delays in payments will result in incomplete services.

  7. Termination of Agreement
    This Agreement may be terminated by SAP if payment is not received in full by the agreed-upon final payment date. Upon termination, SAP will have no further obligation to the Client, and no services will be rendered.

  8. Amendments
    Any changes to the terms of this Agreement must be in writing and agreed upon by both parties.
